Default Identify mystery video mixer?

Hi, I've just joined this forum since I'm hoping someone can help me identify this video mixer. I got it in a mixed ebay lot.

Even though I picked it up in London, it turns out this (and all the other video equipment I got with it) is NTSC.

It doesn't really work properly, giving a very glitchy output. (Yes, I'm using it with other ntsc equipment)

There's no model number or manufacturer anywhere on it, but there is a US address printed on the back:

Anyone recognise this? I'm hoping to one day get it working as it looks like it could be fun.
